Q. Is PROVENT therapy covered by my insurance plan?
A. As PROVENT Therapy is new, it may not be covered by your insurance plan or prescription program. However, the specialty pharmacy that dispenses our product will check your insurance coverage prior to your purchase. Check with your flexible spending administrator; you may be able to use your tax-free, flexible spending account (FSA) funds to cover the cost of PROVENT Therapy.


Q. With the rebate offer, what is the cost of PROVENT Therapy?
A. With the rebate prepaid debit card program, the monthly cost of PROVENT Therapy is reduced from $150 to just $110.

Purchase a minimum of two 30-pack boxes (a 2 month supply) for $300 and you will receive an $80 prepaid debit card. With the rebate program, your monthly cost for PROVENT Therapy is $110.1

For each additional box you purchase, up to a maximum of 12 boxes, you will receive a $40 prepaid debit card (See Table 2). Under the rebate program you can purchase up to a 12 month supply of PROVENT Therapy within a year of the date of your first purchase.

In order to be eligible for the rebate, your first purchase of a minimum of 2 boxes must be purchased between August 1, 2009 and January 31, 2010.

Q. Where can I purchase PROVENT therapy?
A. Your prescription for PROVENT Therapy can only be filled through a specialty pharmacy: 1-888-SLP-WELL or 1-888-757-9355. Your doctor should fax your prescription to the specialty pharmacy (fax: 1-866-916-0583). They will contact you upon receipt of your prescription to confirm your order, collect payment and schedule your shipment.


Q. How do I redeem my rebate prepaid debit card?
A. Follow these 4 steps to receive your PROVENT Therapy rebate:
  1. Purchase any of the Qualifying Products during the Promotion Dates. Circle the eligible purchase on your receipt or invoice. The rebate does not include taxes, processing, or shipping and handling fees.
  2. Complete all information requested on this form accurately and legibly. You may also complete and print the form online at www.rebate-zone.com/provent.
  3. Mail a copy of your receipt, or receipts, and completed offer form to the address listed on the rebate form. Submissions must be postmarked by the Mail-by Date, within 45 days of purchase. Within 6 to 8 weeks, a rebate prepaid debit card will be mailed to you. This prepaid debit card can be used everywhere prepaid debit cards are accepted. The prepaid debit cards will not have cash access at ATMs. Cash advances are available at any member bank branch for no fee and are the sole responsibility of the cardholder. The rebate prepaid debit card will expire 12 months from the issuance date.

  4. Use the rebate prepaid debit card within 12 months of receipt. Prepaid debit cards will expire in 12 months of issuance.



Q. How many months of PROVENT Therapy are eligible for the rebate?
A. The rebate may be used for up to 12 (30-pack) boxes of PROVENT Therapy if purchased within one year from your initial purchase, which must occur between August 1, 2009 and January 31, 2010.
